What Is Trichoscopy?

What Is Trichoscopy? A Closer Look at What Your Hair + Scalp Can Tell Us

August 16, 2026•7 min read

Hair tells us a lot when we know what we’re looking at.

Sometimes the concern is obvious: increased shedding, thinning, breakage, itching, flaking or a scalp that suddenly feels different.

But what we can see with the naked eye is only part of the picture.

That is where trichoscopy comes in.

Trichoscopy is a non-invasive method of examining the scalp, hair follicles and hair fibres under magnification. It allows me to look much more closely at patterns that may not be visible during a traditional hair or scalp consultation.

And for me, that distinction matters.

Because I am not only interested in asking, “Are you losing hair?”

I want to understand:

What is happening at the scalp?
What is happening around the follicle?
What is happening to the hair fibre?
And how are those pieces interacting?

Not Every Trichoscopy Assessment Looks the Same

This is important to understand.

Trichoscopy is a tool and an assessment method, but individual practitioners may use it differently depending on their education, scope of practice, equipment and clinical approach.

My assessment reflects my background in both trichology and cosmetology.

That means I look at the environment the hair is growing from and what happens to the hair after it emerges from the scalp.

I generally divide my trichoscopy assessment into four areas:

Scalp. Fibre. Hair Loss. Measurement + Tracking.

Each gives us a different piece of information.

1. Scalp: Looking at the Environment Hair Is Growing From

Before we start talking about hair growth, we need to look at the scalp supporting it.

During the scalp portion of my assessment, I examine things such as:

  • oiliness or excessive sebum

  • dryness and dehydration

  • flaking and scaling

  • product or environmental buildup

  • possible mineral accumulation

  • redness and irritation

  • visible inflammation

  • follicular buildup

  • changes in the skin surrounding the follicles

  • patterns that may be consistent with certain scalp conditions

  • visible signs that could warrant investigation for infection or microbial involvement

There is an important distinction here: trichoscopy does not replace medical testing or laboratory diagnosis.

I am observing patterns.

If something appears outside my scope or suggests further investigation is appropriate, that information helps me determine when another healthcare professional should join the conversation.

And that is part of a good assessment too.

The goal isn’t to make the scalp fit a diagnosis.

The goal is to observe what is actually there.

2. Hair Fibre: The Part We Sometimes Forget to Assess

This is where my cosmetology background becomes particularly important.

Hair loss gets a lot of attention, but not every reduction in visible hair is coming from the follicle.

Sometimes hair is being shed.

Sometimes it is breaking.

Sometimes it is growing but becoming progressively damaged along the fibre.

And sometimes several of those things are happening at once.

Under magnification, I can examine the condition of the hair fibre and look for patterns of damage that may be associated with:

  • Chemical exposure — colouring, lightening, relaxing, perming and other chemical services.

  • Mechanical stress — brushing, detangling, extensions, tight hairstyles, friction and repeated manipulation.

  • Thermal exposure — blow-drying, flat ironing, curling and other heat practices.

  • Environmental exposure — UV, climate, water quality, seasonal changes and other environmental factors.

  • Everyday hair-care practices — including cleansing, conditioning, styling and product use.

Why does this matter?

Because you cannot solve hair breakage with a hair-loss strategy.

And you cannot assume hair “isn’t growing” simply because someone isn’t seeing additional length.

Hair may be growing perfectly well at the scalp while breaking further down the fibre.

That requires a very different conversation.

3. Hair Loss: Looking for Patterns, Not Just Missing Hair

When assessing hair loss, I am not simply looking for bald areas.

I look at patterns within the hair population itself.

Depending on the concern and assessment, this can include examining:

  • follicular distribution

  • density

  • variation in hair shaft diameter

  • miniaturization patterns

  • empty follicles or follicular openings

  • broken hairs

  • regrowing hairs

  • perifollicular changes

  • redness or inflammation

  • scaling

  • changes between different areas of the scalp

Hair diameter is particularly useful.

Our hairs are naturally not all identical, but changes in the distribution of finer and thicker hairs can provide valuable information when assessing certain forms of progressive hair loss.

Density gives us another piece of the picture.

Instead of relying solely on “I think my hair looks thinner,” we can begin establishing something much more useful: a baseline.

4. Measurement + Tracking: Because Memory Is Not Data

This may be one of the most valuable parts of trichoscopy.

Hair changes slowly.

That can make progress incredibly difficult to judge.

You see your hair every day. Lighting changes. Styling changes. Hair colour changes. Humidity changes. Camera angles change.

And our memory is not particularly scientific.

That is why I measure and document where appropriate.

By recording things such as hair density, hair diameter and standardized trichoscopy images, we can establish a baseline and compare the same areas over time.

That helps us ask better questions.

  • Is density changing?

  • Are we seeing differences in diameter?

  • Is inflammation improving?

  • Has scaling reduced?

  • Are there more visible signs of regrowth?

  • Is the fibre condition improving?

Rather than relying entirely on perception, we have something objective to compare against.

And that becomes particularly important when someone is investing months into changing their hair or scalp routine.

Why I Look at the Scalp, Follicle AND Fibre

Hair doesn’t exist in isolation.

The scalp matters.

The follicle matters.

The fibre matters.

Your health, genetics, environment, styling practices, chemical history and everyday hair care can all influence different parts of that system.

That is why my approach combines trichology and cosmetology.

Trichology helps me investigate the scalp, follicle, hair growth and hair loss.

Cosmetology helps me understand what happens to the hair fibre once it leaves the scalp.

When we put those perspectives together, we get a much more complete picture.

Because sometimes the problem is at the follicle.

Sometimes it is along the fibre.

Sometimes it is the scalp environment.

And sometimes it is a combination.

Can Trichoscopy Be Done Virtually?

In many cases, yes.

I offer trichoscopy assessments in person and virtually, depending on the concern and what needs evaluation.

For virtual assessments, I use specific protocols I have developed to standardize how images and information are collected. Depending on your smartphone, its camera capabilities and the type of assessment needed, I may also arrange for a compatible dermoscope or magnification device to be sent to you so we can capture the images required for a more accurate assessment.

I will guide you through exactly what I need, including lighting, image quality, magnification, specific scalp locations, hair preparation, and how the images should be captured.

In other words, I am not simply trying to assess your scalp from a few random cellphone photos. The correct data collection matters to me. And I do see clients from all over the world.

The protocol matters.

Standardizing how we collect images also gives us something valuable for future comparison. When possible, we can return to the same areas of the scalp using similar conditions to help monitor changes in density, hair diameter, scalp condition and other observable patterns over time.

There are, of course, situations where an in-person assessment, medical evaluation or additional testing may provide information that cannot be obtained virtually. When that happens, we may connect you with a recommended professional or peer who can provide the appropriate assessment, testing or support in your area.

Knowing when to collaborate or bring another professional into the conversation is an important part of responsible trichology care.

Part of my job is knowing the difference and making sure the method of assessment actually fits what we are trying to understand.

Trichoscopy Is Not About Finding One Magic Answer

This is the most important point.

A trichoscopy assessment is not a crystal ball.

It is one piece of the investigation.

  • Your history matters.

  • Your symptoms matter.

  • Your timeline matters.

  • Your health matters.

  • Your hair-care practices matter.

And what we observe under magnification gives us another layer of information to put alongside those pieces.

For me, trichoscopy is ultimately about better observation.

And better observation allows us to ask better questions.

Sometimes that means identifying something we can address through scalp or hair-care practices.

Sometimes it means monitoring changes over time.

Sometimes it means recommending further testing.

And sometimes it means recognizing that another professional needs to become part of the conversation.

That is exactly what a good assessment should do.

Understand the follicle. Protect the fibre. Support the person.

Because when you can see more clearly what is happening, you can make much better decisions about what happens next.
